This artwork is about Papa Nihil where he's still alive stealing people's hearts and turning them into masked Nameless Ghouls, only to integrate them into his own body later, thereby extending his life (just like Kakuzu in Naruto). Papa is traveling alongside Cardinal Copia who performs the rituals (Hidan style xD).
